1 Hart, 1 Mind, 1 Future

Hartlepool's recognised Parent Carer Forum, established 2008 - a group of parents/carers of children with SEN and disabled children aged 0-25 who work alongside the local authority, education, health and other service providers to help plan, commission, deliver and monitor support for children and families. Works closely with Hartlepool Borough Council's Short Break Lead, and jointly funds a Parent Carer Link Worker with Hartlepool Carers.
